 Online registration and Roster/Waiver form All participants in individual programs or team leagues, tournaments, and other activities must sign a roster form prior to using the Family Sports Center. Parents must also sign the roster form for youth prior to playing. We use the same form for both individual and team programs. Individuals need only fill out a three line section at the bottom of the front page of the wavier form. Teams must fill out the top section and each team member fills out a three line section at the bottom or on page two. Blue Chip Football Summer 7 on 7 Clinic Position and Performance Training for 5th thru 8th Grade Boys Lead Instructor: Patrick Johnson Patrick went to Potosi High in Missouri where he was an All-State Athlete in 3 sports. He was Mr. Football in the state of Missouri. At Purdue University he was a 4 year letterman, Honorable Mention All- American and also played in the Blue/Gray game. After college Patrick played 3 years in the NFL with the Miami Dolphins and Green Bay Packers.
