HIGH SCHOOL SOCCER 2011/2012 If you want to join please fill out the online registration or mail in registration forms on the soccer home page(payment does not need to be made upon registration) All participants in individual programs or team leagues, tournaments, and other activities must sign a roster form prior to using the Family Sports Center. Parents must also sign the roster form for youth prior to playing. We use the same form for both individual and team programs. Individuals need only fill out a three line section at the bottom of the front page of the wavier form. Teams must fill out the top section and each team member fills out a three line section at the bottom or on page two League Play: 2011/2012 $500 per team (14 player max per roster) Thursdays 5-9pm Session 1: November 10th - January 5th Session 2: January 19th - March 8th
